VNA/Hospice Becomes Subsidiary of Pocono Health System

East Stroudsburg, Pennsylvania (May 8, 2014) –VNA/Hospice has officially become the sixth operating subsidiary under the Pocono Health System (PHS) umbrella. This partnership will result in coordinated and integrated care to all of the patients they serve. This includes patients in Luzerne County, Monroe County, Northampton County, and Pike County. The new partnership will result in an effective continuum of care from the hospital to home or hospice care thus improving patient quality, safety, and satisfaction.

“We are very excited about this partnership which will strengthen both PHS and VNA while also fulfilling our vision of building a healthier community,” said Jeff Snyder, Interim President and CEO of Pocono Health System/Pocono Medical Center. “The partnership offers a greater continuum of services to our community and further demonstrates our focus on doing what is best for patients.”

Mark Hodgson, CEO, VNA/Hospice added, “Since 1950, VNA/Hospice has been providing home care and hospice services to area residents and together we look forward to looking at ways to enhance the care provided to our patients.” Best practices identified by PHS and VNA/Hospice will be incorporated throughout the continuum of care resulting in an enhanced coordination of care and a more effective delivery model.

“We enthusiastically welcome all 85 dedicated employees of VNA/Hospice to the PHS team,” states Jeff Snyder, Interim President and CEO of Pocono Health System/Pocono Medical Center. The Hospice House, an acute in-patient facility for the terminally ill with 6 beds will also become part of PHS’ Palliative Medicine Program.
